Practice Partner Clients Play Major Role in AAFP’s TransforMED Initiative

08/22/2006 Seattle, WA — Practice Partner, a leader in the development of electronic health records (EHR) and practice management software, today announced that five of the 18 intervention practices in the American Academy of Family Physicians’ (AAFP) TransforMED project are Practice Partner clients.

TransforMED is a national demonstration project of the AAFP focused on transformative practice redesign, including the use of electronic medical records, e-mail, web and voice communication. The $8 million effort proposes to change the way primary care is delivered by improving quality and access as well as workflow processes.

The 36 practices in the two-year project were chosen from among more than 300 applicants and vary both geographically and demographically. The practices are divided into two groups of 18; the intervention group will receive support from practice redesign experts and share best practice information with other practices; the control group will only be provided access to practice improvement tools and services without other assistance. All five of the Practice Partner sites are in the intervention group.

Practice Partner is a long-time leader in practice-based research. The company hosts its own research through the Practice Partner Research Network (PPRNet), the first practice-based research network that links physicians who use electronic health records. PPRNet provides practices with benchmarked performance reports on approximately 80 quality indicators.

A final TransforMED report is expected to be released in 2009; AAFP has committed to sharing key data points and lessons on an ongoing basis.
