Practice Partner Completes Continuity of Care Record Integration; Will Demonstrate at TEPR

05/17/2004 Seattle, WA — Practice Partner, a leader in the development of electronic medical records and practice management software, today announced the successful integration of the Continuity of Care Record (CCR) with Practice Partner Patient Records. Practice Partner will demonstrate this capability at Towards an Electronic Patient Record (TEPR) in Fort Lauderdale on May 17-21.

The CCR is an initiative sponsored by groups that include the Medical Records Institute, the Healthcare Information and Management Systems Society, the American Medical Association, the American Academy of Family Physicians, the American Academy of Pediatrics and the Patient Safety Institute. The CCR is a standard for a summary of personal health information that clinicians can send when a patient is referred, discharged, or transferred. Patients can carry this standardized health summary with them to promote continuity, quality, and safety of care.

Practice Partner will participate in the CCR Demonstration Project at TEPR to show how the personal health information contained in a sample record using the CCR standard can be quickly and accurately loaded into Practice Partner Patient Records. Patient Records automatically stores information from the record such as past medical history, family and social history, allergies, medications, recent procedures, and lab results. Patient Records loads information into the appropriate section of the new or existing patient chart, without the need for manual data entry, and maintains a complete copy of the CCR record.
